Midland Care PACE

Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a health care program that helps people at risk for needing nursing home stay in their own homes and communities.

Midland Care PACE may be an option for anyone who:

  • Is 55 years old or over.
  • Lives in the Midland Care PACE service area in Douglas, Franklin, Jackson, Jefferson, Johnson, Leavenworth, Lyon, Marshall, Miami, Nemaha, Osage, Pottawatomie, Shawnee, Wabaunsee, and Wyandotte Counties in Kansas. If you live outside the Midland Care PACE service area, PACE may still be available from another provider. To find out, visit the National PACE Association online.
  • Qualifies for a nursing home level of care, meaning someone needs extra help to live at home safely, according to eligibility requirements determined by the State of Kansas.
  • Able to live safely in the community with the help of PACE services
