Hope Family Care Center

Hope Family Care Center is a primary-care doctor’s office providing quality, personal healthcare. All patients are welcome. We accept Medicaid, Medicare, and commercial insurance. Uninsured patients are charged a small fee. If you live in the zip code areas 64109, 64127, and 64128 and are not able to get enough food, transportation, housing, or medical care, come to Hope Family Care Center and find friends who care for you.

Patient Services

Adults and Pediatric Primary Care: 
We offer complete primary and preventive care services for the entire family. All ages are served from newborn babies to the elderly. Annual sports and school physicals, employment physicals Immunizations/vaccinations for adults and children

Chronic Disease Management:
We provide ongoing chronic disease care, education, and monitoring including asthma, diabetes, hypertension, and obesity among others.

Women’s Health and Prenatal Care:
We offer routine care for women and expecting mothers including annual well woman exams, birth control, prenatal care and sonograms.

Acute Care:
We provide immediate treatment for minor injuries, illnesses and urgent medical conditions such as flu,colds, bronchitis, cuts, sprains, and more.
