National POLST

National POLST is program focused on efforts to provide best practice guidance about the POLST process and form as well as education throughout the U.S. They are currently housed as a program at the Louisiana Health Care Quality Forum, an independent 501(c)3 non-profit organization. National POLST organizes the effort to standardize the POLST process, form and education throughout the U.S., since POLST varies from state to state.
